§ 14-40. License application to commissioner of finance; contents.

Application for a license hereunder shall be made in writing to the commissioner of finance, by the person, general or limited partnership, corporation or limited liability company to be licensed, on forms to be furnished by said commissioner of finance. The application shall contain such information as to name and address of the applicant, matters affecting public health, and such other information as the commissioner of finance may reasonably require.
